    專業護理團隊

    The home care service team is led by specialist nurses who serve as case managers. Working with medical staff, rehabilitation therapists, and patients and their families, they meticulously plan home rehabilitation and care programs to meet the patient's needs and provide the most suitable nursing services. Through a remote health monitoring system, case managers provide timely reminders to patients and their families; and in-home care services are provided when necessary.

    居家護理服務醫護團隊由專科護師擔任個案經理，聯合醫護人員、康復治療師與患者及其家屬，配合患者需要，貼心策劃居家康復護理計劃，提供最合適的護理服務。透過遠程健康指數監察系統，由個案經理負責，適時提醒患者及其家人；有須要時，並適時提供到户護理服務。

    The service aims to provide comprehensive and continuous medical and nursing care through a home care model, reducing the time, manpower, and emotional burden on patients and their families traveling to and from the hospital. It prevents interruptions in medical services due to discharge, reduces unnecessary hospitalization costs, and lowers the risk of infection during hospitalization.

    服務職志以居家護理模式提供完整連續性之醫療護理服務，減輕病人及家屬往返醫院所花費的時間、人力、精神，不致因出院而造成醫療服務中斷，能減少不必要的住院所耗費之醫療費用及減少病人住院中引起的感染機率。
